Key Responsibilities:

Supporting individuals within their own homes and in the community

Providing personal care while maintaining dignity, privacy and respect

Supporting with day-to-day activities and promoting independence

Supporting individuals with mobility and accessing the community

Providing emotional and practical support

Supporting individuals with complex and / or clinical care needs following appropriate training

Supporting with medication in line with training and care plans

Following individual care and risk management plans

Maintaining accurate care records and reporting any concerns

Working alongside families, healthcare professionals and the wider care team. This is a FIELD-BASED CARE position. You will be travelling to individuals' homes and care settings to provide direct support. You will need to be 18+ due to insurance and have access to a vehicle.

Applicants must also be comfortable providing personal care and working flexible hours according to the needs of the service. No sponsorship is available for this position. Applicants must already have the appropriate Right to Work in the UK.
